Open Campus Lunchtime Policy Notification

Dear Pacific Collegiate School Parents and Guardians:

Pursuant to Section 44808.5 of the Education Code, the governing board of the Pacific Collegiate School ("PCS") has decided to permit its pupils in grades 9-12 to leave the school grounds during the lunch period.

Please be advised that neither PCS, the Santa Cruz County Office of Education, any other public entity associated with the school, or any agent, officer or employee thereof, shall be liable for the conduct or safety of any pupil or any damages arising from any accident, illness, injury, or death during such time as the pupil has left the school grounds pursuant to this open campus lunchtime policy.

THIS PARENTAL/GUARDIAN NOTIFICATION FORM MUST BE SIGNED AND SUBMITTED TO THE SCHOOL PRIOR TO YOUR CHILD LEAVING CAMPUS DURING THE LUNCH-PERIOD. NO PERMISSION FOR A STUDENT TO LEAVE THE SCHOOL CAMPUS DURING THE LUNCH-PERIOD WILL BE GRANTED OVER THE TELEPHONE.

I HEREBY ACKNOWLEDGE AND ACCEPT FULL RESPONSIBILITY FOR THE RELEASE OF SAID STUDENT FOR THE DURATION OF THEIR ENROLLMENT AT PACIFIC COLLEGIATE SCHOOL AND WAIVE ANY AND ALL CLAIMS AGAINST PCS, THE SANTA CRUZ OFFICE OF EDUCATION, ANY OTHER PUBLIC ENTITY ASSOCIATED WITH THE SCHOOL, OR ANY AGENT, OFFICER OR EMPLOYEE THEREOF, FOR ANY ACCIDENT, INJURY, ILLNESS, OR DEATH OCCURRING BY REASON OF THE RELEASE OF THE STUDENT FROM THE SCHOOL CAMPUS DURING THE LUNCH-PERIOD.
