This Notice is given with reference to Pacific Collegiate School, a California nonprofit corporation, located at 3004 Mission Street, Santa Cruz, California (hereinafter "School Property").

1. The School Property is located in an area zoned for industrial uses by the City of Santa Cruz, and the School Property is near and downwind of industrial lands and industrial operations.
2. Persons on the School Property may be subject to inconveniences, discomforts, or adverse effects arising from the lawful and reasonable use and activities of such industrial operations, including but not limited to noise, odors, fumes, dust, smoke, air emissions, and operation of machinery during any 24-hour period.
3. By signing below, the undersigned acknowledges receipt of this Notice on the date indicated.

NOTE FROM PCS ADMINISTRATION:

We ask our parents/guardians to sign this acknowledgment every year as part of our agreement with the City of Santa Cruz when they granted us a Use Permit to occupy our building. However, to our knowledge, there are no environmental hazards in our vicinity. As part of the process of purchasing our building, we conducted a full Phase One Environmental Site Assessment; no environmental problems or concerns were cited.
